on with your life! My name Dr. Christopher J. Connelly, DC. I have been in Chiropractic private practice for over 10 years and over this time I have had over 40,000 patient visits. I have also performed over 200
independent medical reviews, impairment ratings, case summaries or disability reports. "A good diagnosis
is the first step to a good treatment" So the first step in treatment at my office is obtaining an anatomical
diagnosis concerning the cause of your pain. The next step is to get you out of pain as soon as possible. We will then work
to correct any problems utilizing chiropractic, physiotherapy, disc decompression, exercise, injury avoidance, home care,
massage and diagnosis specific procedures. I also understand that sometimes a patient may need to be referred to a medical
specialist or surgeon.
OBJECTIVE

provide specialized, professional and diagnosis based services to patients, insurance companies, doctors and legal professionals,
for back pain, neck pain disc herniations , disability and other impairments. When you are in pain or have been injured, choosing a doctor is one of the most important decisions you can make.

FUNCTIONAL SUMMARY
My goal for new patients is to get a
diagnosis concerning the cause of your pain and then to get you out of pain as quickly as possible. Depending on your diagnosis
we will work to correct any structural problems. We believe in a family and friendly atmosphere. We
want this to be a wonderful experience for you, every member of your family and anyone you refer to our center. In todays
healthcare environment there is a growing need for outcome based measures to control cost and promote quality based care /
treatment. Theses services are based on nationally recognized guidelines such as the AMA Guide to the Evaluation of Permanent
Impairment, AMA Guide to Disability, Croft Guidelines, The Quebec Task Force, and other professionally accepted guidelines.

SUMMARY OF QUALIFICATIONS
I have been in clinical practice since 1998. Over this time I have treated approximately 800+ trauma cases, 40,000 patient
visits and 200+ cases for second opinions, impairment / disability ratings, case summaries or independent evaluations.
